A paid first engagement, required before any retainer. Two hours in your parent’s home, records collected and reviewed, existing providers contacted, and the completed Continuity File and a written baseline assessment delivered within ten days. Some families stop here, with a record they did not have before.
The levels differ by rhythm rather than by feature — how often we attend appointments, how often we are in contact, and how much coordination the situation actually needs. Most families sit in the middle. We will recommend a level; you are not asked to guess.
One attended appointment, start to finish, with a written account the same day. Or a defined two-week engagement beginning at hospital discharge, when the days after a discharge are deciding the year. Most retained families begin with one of these.
Three things: how many physicians are involved, how stable your parent is at the moment, and how far away you live. A stable parent with one cardiologist and a daughter in Friendswood needs a fraction of what an unstable parent with five specialists and a son in Boston needs.
Nothing is bundled that you will not use, and the level is reviewed rather than assumed — if the situation settles, we say so and the fee comes down.
We are private pay only. We do not bill Medicare, Medicaid, long-term care insurance or any other insurer, and our fees are not reimbursable. That is deliberate: it means the nurse answers to your family and to no one else.
Most families find the monthly fee is a fraction of what one avoidable readmission, one unnecessary move, or a week of unpaid leave and two flights would have cost.
The fee schedule is one page. We do not publish it because the right level depends on what is actually happening, and a figure without that context tends to mislead rather than inform.
